Instructions To Transfer Your Child’s Care

Sunshine Pediatric Clinic is closing June 30, 2024.  Dr. Craddock is relocating to support her two kids’ career goals.  It has been an honor to serve the children and families of East Hawaii for almost 10 years. Telemedicine appointments are available now through June 30, 2024.  

All patients will be automatically reassigned to a new provider on July 1, 2024.   Your insurance will contact you with the new assignment.  We can only provide access to your medical records and vaccines online.  You must visit www.sunshinepediatricclinic.com/services/ to create an account and log into the patient portal for your child.   We only provide medical records for care provide by Sunshine Pediatric Clinic providers.   We cannot provide historical medical records from other providers.

Ohana, United, and AlohaCare patients should contact their insurances right away for reassignment. Note that if you are traveling in August 2024, you must submit request 30 days prior to travel date with these insurances, so make sure you establish care with your new provider by July 1, 2024. Ultimately your new provider is your choice.
